  • Is there a clearly defined use case and success criteria?
  • Have stakeholders been involved across the lifecycle?
  • Is risk appetite explicitly defined?

  • Is there monitoring for drift?
  • Are fallback mechanisms in place?

6. Transparency and Explainability

  • Is the system’s purpose documented?
  • Can decisions be explained to stakeholders?
  • Are users informed they are interacting with AI?

7. Human Oversight

  • Is there human-in-the-loop governance?
  • Can decisions be overridden?
  • Are escalation pathways defined?

8. Technical Security

  • Is the system protected from adversarial attacks?
  • Are data pipelines secure?
  • Has penetration testing been conducted?

9. Risk Monitoring and Reporting

  • Are risks tracked continuously?
  • Is there a central AI risk register?
  • Are incidents analysed and learned from?

10. Deployment and Lifecycle Management

  • Has the system been tested in real-world conditions?
  • Are retraining processes controlled?
  • Is there an end-of-life plan?
